One patient at a time. Your consultation, your operation and your follow-ups — handled personally by Dr. Akçay, never delegated.
Tear-drop implants · six months
A breast needs six months to look its best, so read the early photographs with that in mind. Every case below is labelled with its healing stage, and many name the implant by brand, shape or volume. The strip also refuses to skip the awkward chapters: case 06 sits in its first weeks with surgical tape still on, cases 07 and 09 were photographed on the day of surgery, and case 08 is still carrying its bruises.

“There is no perfect breast — there is the breast that belongs on your frame. The right size is measured, not wished for: your tissue, your chest wall and your life decide it with us at the consultation, never a catalogue. Whether that means an implant, a lift, a reduction or undoing an old decision, I plan it with you and perform it with my own hands. Getting it right the second time is the same craft as getting it right the first — the breast must belong to the body it lives on.”

Verify his fellowship in the EBOPRAS registryDr. Akçay himself. One pair of hands carries your case from the first photo review, through the consultation and the operation, to every follow-up that comes after. Nothing in that chain is delegated, and his European Board fellowship can be checked independently in the EBOPRAS registry linked above.
Because it is decided in a room, not over WhatsApp photos. At your in-person consultation you try sizers in a surgical bra while Dr. Akçay takes tissue-based measurements: breast width, skin stretch, chest-wall shape. The implant has to fit your chest wall; the volume that looks effortless on one frame looks borrowed on another, and the measurements tell you which is which before anything is decided.
Let’s name both, calmly. BIA-ALCL is a rare lymphoma associated overwhelmingly with certain textured implant surfaces; breast implant illness (BII) describes symptoms some women report, and both are taken seriously here rather than waved away. Dr. Akçay uses CE-marked implants from Motiva and Mentor, and the surface and shape of yours are decided openly with you at the consultation, with the trade-offs of each explained first. After surgery you receive the implant’s ID card, its “passport”, with serial numbers and specifications. It is yours to keep, so any doctor, anywhere, in any year, knows exactly what you carry.
Aftercare keeps its appointments. Before departure your incisions are examined, your supportive bra is fitted, and you are personally cleared to fly. Then come photo reviews at weeks 2, 6 and 12, and again at months 6 and 12, all inside the same WhatsApp conversation you opened on day one. You also travel home carrying your complete medical records in English, so your own doctor can read exactly what was done.
CE-marked Motiva or Mentor implants, round or tear-drop, sized with sizers and tissue measurements at your consultation and placed through a short incision in the fold where every bra sits.
Ask about augmentationFor breasts that have descended after pregnancy, weight change or time. Reshaped and repositioned using your own tissue, the nipple lifted back to where it began, with no implant needed where your volume is enough.
Ask about a breast liftWhen volume has gone and position has followed it, lift and implant happen in one operation, one anaesthesia, one recovery. The plan photos most often reveal, and the one most often underestimated.
Ask about lift with implantsWeight off the neck, shoulders and back: the breast reduced, reshaped and lifted in the same surgery. Patients describe this one in terms of posture and sport, not mirrors.
Ask about reductionOlder, mis-sized or uncomfortable implants removed and replaced, the capsule addressed and the breast re-lifted where needed, in one operation. Often this is the surgery that undoes a decision made years ago — case 09 in the gallery is exactly this, photographed on the day.
Ask about implant exchangeA constricted breast base many women have never had named to them. Released and reshaped, usually with an implant, so the breast can finally take its natural form. A diagnosis first, then a plan.
Ask about tuberous correctionA few phone photos and your goals. Dr. Akçay reviews them personally and you receive an honest assessment, which sometimes reads “a lift, not implants” or “you don’t need surgery at all.”
Operation recommendation, a written all-inclusive quote with nothing hidden, and a video consultation with Dr. Akçay himself before you commit to anything. Nobody chases you for a deposit, and nothing here is on a timer.
A driver holding your name meets you at Istanbul Airport, and a VIP car brings you to your hotel. Settle in, eat lightly, and lay out a loose, front-opening top for the days ahead; after breast surgery you will not want anything that pulls over the head. Your coordinator stays one message away on the thread you already know.
In-person examination and sizing come first: trial implants in a surgical bra, plus tissue-based measurements. Pre-operative tests follow (blood panel, ECG, anaesthesiology review) before surgery in a fully equipped, Ministry of Health–licensed hospital in Istanbul with a dedicated anaesthesia team. You stay one night under nursing care.
Rest at your hotel between scheduled in-person checks. Your supportive bra is fitted and adjusted, dressings are reviewed, and medication and daily guidance are handled for you. Far from home, you still have a team on hand rather than a search engine.
Dr. Akçay examines the incisions and the early shape, answers everything, and only then are you cleared to fly home, carrying your complete medical records in English.
Photo reviews at weeks 2, 6 and 12, then months 6 and 12, on the same WhatsApp thread. The settled, final shape is judged at six to twelve months, and Dr. Akçay looks at it with you.

Plan for 6–7 nights. Surgery is usually on day two with one night in hospital; your final check falls around day six or seven, and you fly home only after Dr. Akçay has examined you and cleared you.
A driver holding a card with your name on it, waiting at arrivals; the VIP transfer is part of your package, not an extra. Your coordinator confirms every pickup with you the evening before, so you always know who is coming and when. The same door-to-door arrangement covers each hospital and clinic visit during your stay.
For most augmentations, a short incision in the inframammary fold, the natural crease under the breast, hidden where every bra and bikini sits. A lift adds a fine line around the edge of the areola (periareolar); a full lift or reduction adds a vertical line below it. Dr. Akçay shows you each scar on real healed photos at your consultation, so you decide knowing exactly what fades to what.
CE-marked implants from Motiva and Mentor. Shape, projection and surface are weighed against your measurements and agreed with you at the consultation, and your written quote names the exact device rather than saying “implants included”. After surgery you receive the implant ID card, your implant’s passport, listing manufacturer, size and serial numbers. Keep it; any doctor anywhere can identify exactly what you carry.
In a fully equipped, Ministry of Health–licensed hospital in Istanbul. Pre-operative tests come first (blood panel, ECG and an anaesthesiology review), a dedicated anaesthesia team looks after you throughout the operation, and you spend one night under nursing care before returning to your hotel.
Usually, yes. The fold incision avoids the milk ducts entirely, and sensation is preserved in most patients. But the honest answer is individual: it depends on the operation (a reduction changes more than an augmentation) and on you. Dr. Akçay tells you plainly at your consultation what your specific plan means for both, before you decide anything.
There is a volume your tissue can carry gracefully, and it is measured, not guessed: breast width, skin elasticity, chest-wall shape. Dr. Akçay gives you that range with sizers in your hands; if you want to go beyond it, he explains exactly what the trade-offs look like rather than simply agreeing. The results above are the register he works in.
No. Modern implants are not on a timer. They are monitored, not automatically exchanged: your photo reviews, routine checks at home and the ID card in your wallet keep track of exactly what you have. If implant and breast remain healthy, the implant stays; if anything ever changes, you message the same thread first.
A useful shorthand: lost volume points to an implant; a nipple that sits low points to a lift; both together point to mastopexy-augmentation in one operation. The real answer comes from your photos and your in-person examination, and sometimes it is “a lift alone”, which costs less than the implants you thought you needed.
Surgery and anaesthesia, your named implants, the hospital night, your recovery hotel, all VIP transfers, your coordinator, all post-op checks in Istanbul, your surgical bra, medication, and the 12-month follow-up programme. All of it sits in one written, itemised quote, and the total you read is the total you pay.
You leave hospital in a supportive surgical bra, worn day and night for the first weeks. No underwire until Dr. Akçay clears it. Walking starts immediately; light exercise typically returns around week four, chest training and high-impact sport later. Your dates are set in writing at your final check, not guessed from a blog.
Say so on the thread, with photos. The 12-month photo-review programme exists precisely to catch concerns early, while they are easiest to assess. If what you show is normal healing, you will be told so, and why. If a finding genuinely warrants treatment, Dr. Akçay tells you honestly and plans the correction with the same written clarity as the first operation.
Volume and economics explain it, not shortcuts. Few cities on earth perform more surgery than Istanbul, and at that scale hospitals, staff and devices simply cost less to run than they do in London or Berlin. What you are buying is unchanged: a European Board-certified surgeon delivering world-class care in a country where it costs less to provide. The saving alone would cover your flights many times over.

A final word
A decision like this rewards patience, so weigh the surgeons themselves rather than the price tags. Ask us anything, and if the honest answer for your body is a smaller operation, or none at all, Dr. Akçay will tell you that too.
